The We Do It For You Name Change Kit: $99.95
The We Do It For You Name Change Kit contains all of the same basic forms and procedures as the Do It Yourself Name Change Kit but without the actual hassle of doing it all yourself. With the exception of contacting the DMV (which requires you to appear in person in most instances), you merely (i) follow the instructions under the Getting Started page, (ii) complete the name change forms, (iii) complete this Limited Power of Attorney (which needs to be notarized) to authorize us to process your last name change on your behalf and (vi) mail all of the documents back to us c/o Curt A. Kramer, Change Your Last Name, 624 Buttonwood Circle, Naperville, IL 60540. We take care of the rest.
